Australian Better Health Inititiave (ABHI)

The ABHI program commenced in 2006 as a four year initiative to strenghten the health systems focus on promoting good health and reducing the burden of chronic disease.

Five prority areas were indentified:

  • 1. Promoting Healthy Lifestyles
  • 2. Supporting Early Detection of Lifestlye risks factors and chronic disease
  • 3. Supporting lifestlye and risk factor modification
  • 4. Encourage active patient self management
  • 5. Improve integration and coordination of care

Divisions throughout NSW have been invited to assist with priority area five.  The CWDGP project specifically deals with improving integration of diabetes services across the Division. To achieve this, we aim to assist general practice and other health providers  to develop solutions to reduce duplication and allow for a 'seamless' approach to patient care.

In addition, the CWDGP will assist the HealthOne centres in Blayney, Molong and Rylstone with integration of services for patients with disbetes.
