The following screening tools have been compiled as part of a Comorbidity Education project, funded by AGPN, to assist in identification of comorbid mental health and drug and alcohol presentations in General Practice.  These evidence-based screening tools are available to be downloaded for use in primary health care settings.

DASS Tool

The DASS Tool asks a series of questions for completion by the patient (in either DASS 21 or DASS 42 format) on the Depression, Anxiety, Stress scale. Note that DASS 42 is more regularly known as simply "DASS".  DASS 21 is an abbreviated version.

The online version below allows for the copying and pasting of results into a Clinical Application.  The practitioner simply selects "Show Results for copy and paste" to allow pasting into the clinical record ( eg progress notes or within a letter ), or alternatively print the result for placement in the patients file.

Please note that the online DASS tool below does not ask for any information that indentifies the patient, the association of the results to an individual is left to the practitioner to place results within the appropriate clinical record in their own practice.

The tool does not save any results.

The K10 chart is already installed in most GP Clinical Applications today

  • In Medical Director, Select New letter, Click the "Supplied" tab.  The K10 chart is listed as "Mental Health - K10"

Questionnaire for alcohol, tobacco and other drugs.

We suggest that you load this questionnaire into your "Patient Education" section within Medical Director.  It is a 12 page pdf document that is not suitable for inclusion in patient letters.  It is best hand completed by the patient, and scanned back into the patient record when completed.  Alternatively, you can always download the file from this website.
