Privacy Notice
In compliance with the Privacy Amendment (Private Sector) Act 2000, the NSW Central West Division of General Practice has prepared this Privacy Policy to describe the way and circumstances under which personal information is collected, stored, used and disclosed by the NSW Central West Division of General Practice. The Policy is intended as a guide to the staff and/or clients and/or patients and/or members of the NSW Central West Division of General Practice and for the advice of the broader community.

The NSW Central West Division of General Practice will provide a copy of this Policy to all members of staff and will train staff in the appropriate handling of personal information. Staff members are required to sign a confidentiality agreement on commencement of employment with the Division signifying that they have read and understood this policy.
This policy is a public document and access to it will be granted on request.
Privacy Complaints Handling
Any complaints in relation to the NSW Central West Division of General Practice’s handling of personal information should be directed to the Privacy Officer. In most cases the complainant will be asked to lodge their complaint in writing.
Unless a complaint can be dealt with immediately to the satisfaction of both parties, the NSW Central West Division of General Practice will provide a written response to the complaint within 30 days of it being received.
If an individual believes their complaint has not been appropriately handled by the NSW Central West Division of General Practice, they should contact the Office of the Federal Privacy Commissioner, Privacy Hotline 1300 363 992 (local call charge) or via www.privacy.gov.au
18.1. Collection
The NSW Central West Division of General Practice will only collect personal information necessary to undertake its programs, activities or functions.
Personal information about an individual will only be collected by lawful and fair means and directly from the individual wherever possible.
The name and contact details of the NSW Central West Division of General Practice and an appropriate member of staff will be supplied to every individual who provides personal information.
The NSW Central West Division of General Practice will ensure that each individual providing personal information is informed about and understands the purpose of collecting the information, to whom or under what circumstances their personal information may be disclosed to another party, and how they can access the information held about them by the NSW Central West Division of General Practice.
The NSW Central West Division of General Practice will ensure that individuals who are asked to provide personal information understand the consequences, if any, of not providing that information or of providing incomplete or inaccurate information.
18.2. Use & Disclosure
The NSW Central West Division of General Practice will ensure that personal information will only be used for the purpose it was collected, or that would reasonably be expected by the individual providing the information.
18.2.1 If the identified information is to be used for a secondary or unrelated purpose, such as data analysis or research, we will obtain informed consent from the individual.
Individuals will be given the opportunity to refuse such use or disclosure.
If an individual is physically or legally incapable of providing consent, a responsible person# may do so.

Under normal circumstances the NSW Central West Division of General Practice will provide an individual with access to their personal information within 30 days of receiving a request for access.

18.6.2. A small administration fee may be charged for the provision of access and/or copies of personal information. This fee will be determined on a cost recovery basis.
18.6.3. Individuals will be provided access that is appropriate to their particular circumstances.
18.6.4. Individuals will be provided an opportunity to discuss their personal information with an appropriate member of staff when access is sought.
18.6.5. If an individual believes that information held by the NSW Central West Division of General Practice is inaccurate or incomplete, the NSW Central West Division of General Practice will take steps to amend or correct the information.
